I/we have read CLC’s Photo & Video Policy on reasons for photography/videography, special provisions for minors, and the procedure for removal of individual photos or video segments. *

I do not want a photo or video of my child(ren) to be posted online or otherwise displayed (e.g. church announcement videos or newsletter). I understand that Christian Life Center will make every effort to exclude my child(ren) from any individual or small group photos or videos. I understand children may still be included in large group or crowd shots without identifying information and that I always request that such any photo or video scene be removed from display.
